Targeted Case Management is where emotional support meets real-world problem solving. We sit beside you to map out what you need - housing, food, childcare, transportation, benefits - and then walk step by step through how to get there.
A core part of this service is Benefit Application Assistance. Many families feel overwhelmed by forms, documentation, and phone calls. We slow the process down, explain what each question means, help you gather paperwork, and stay with you through submission and follow-up. This can include support with financial assistance, food benefits, disability-related programs, or other public supports.
Along the way, we coordinate with your therapist and other providers so your emotional health, recovery, and practical needs are addressed together. The goal is not just to "get through a crisis," but to use each resource as a stepping stone toward greater stability, dignity, and long-term self-sufficiency.
Share your needs and questions, and our team will respond promptly to explore support options together with you.
